Apex Console boots with black screen and shows EFI Shell prompt

Symptoms/Issue

Apex console boots to a screen with a black background, yellow and white text that reads "Device mapping table", and a prompt that reads "Shell:\>"  This may have happened after a Blue Screen of Death event.

Or, the text on screen may say something like "EFI Shell version".
