Fader value drops or gets erratic when at 0% or 100%

Symptoms/Issue

When moving the fader to full 100% but the intensity value drops to 0% or gets erratic rather than staying at 100%.

The fader more than likely needs to be replaced.
Test by swapping the connections with a known good fader next to it if possible.
